SGA CLUB APPLICATIONS
New Student Organization Application - required for any new club or student organization seeking approval from the SGA as required by the Office of Student Affairs. To view criteria by which applications for new student clubs or organizations are reviewed, please review the New Student Organization Criteria for Approval
Club/Class Meeting Application - must be submitted to Student Affairs at least 2 weeks prior to your club or class meeting in order for Student Affairs to approve the meeting. This form also serves as your request for a room reservation on campus. Contact Diane Dougherty (DougheDE@evms.edu) in Student Affairs with any questions or concerns related to using funds from a club/class account.
Club/Class Events & Fundraising Application - Must be submitted to Student Affairs at least 4 weeks prior to your club/class event. Contact Joann Bautti (bauttij@evms.edu) in Student Affairs with any questions or concerns about this process, use of club/class funds, and/or alternative funding opportunities.

SGA CLUB FINANCES
SGA Loan - loans available to new programs prior to Student Fee processing.
SGA Grant Application - student organizations can request funding of up to $250 from the SGA to further their club's mission. To view the guidelines for requests of funds from Student Affairs related to a community service initiative, please review the Student Fund Guidelines.
Student Fund Application - application for funds from the Student Fund controlled by Student Affairs - these funds may be allocated for community service initiatives that further EVMS' vision to be the 'most community oriented medical school in the nation.' For any questions, please contact Joann Bautti (bauttij@evms.edu).
Community Outreach Grant - Community Outreach is defined as an activity conducted outside a traditional, institutional, or framed population with the purpose of providing direct services, referrals, programs, or education. The Community Outreach Mini Grants are available to students who wish to conduct a service, program/project, or activity in the community. The following programs are eligible to apply: MD, Art Therapy, Medical Masters, Biomedical Science, Biomedical PhD, SA, MPH, MPA, MCHA, and PATHA. Applications need to be submitted 30 days prior to an activity/event. Applicants will get notification of approval or denial 1 week after submission and review by a committee.
Tax-Exempt Form - this form is required to establish EVMS Tax Exempt Form tax-exempt status for purchases intended for EVMS purposes. For more information about the tax exempt form, please contact Diane Dougherty (DougheDE@evms.edu).

Student Affairs Travel and Publication Grant
- Overview for Travel Funding - for students looking to travel to academic conferences or for other EVMS-related reasons, this document provides an overview of what financial assistance options are available.
- Student Affairs Travel Grant - To apply for any institutional conference travel or publication funding, please complete and submit this form to the Office of Research, EVMSResearch@evms.edu (HP students) or the Medical Student Research Office, MSR@evms.edu (MD students). They will assess which funding options are available to you.
- Please note: For support of conference travel fees, students must be presenting research / For support of publication fees, students must be first-author of said publication. Conference travel funding is a pre-approval process; students must complete funding requests at least four weeks before their conference dates.

The EVMS Student Government Association exists to promote leadership, service and a sense of solidarity between the various classes and organizations at Eastern Virginia Medical School. As the head of student government, our role is to represent the EVMS student body to the staff, faculty, administration and alumni. Our strategic goals are to better our school, our community and our society at large by encouraging social responsibility and supporting active participation in student government, student organizations and service to our communities. We believe in an open culture of progress and change; and, ultimately as a group, we believe that we can foster and help sustain efforts that lead us all towards a better and brighter future.

EVMS SGA SUBCOMMITTEES
MD Subcommittee
Chair: Student Body President
Membership in this subcommittee is open to all EVMS Students. All topics related to MD-only issues shall be discussed within this subcommittee. The MD Subcommittee shall be responsible for the annual election of each new MD class’ AAMC Representative as well as filling any AAMC Representative vacancies. The Subcommittee shall be responsible for maintaining communication and report findings to the EVMS Vice Dean of the School of Medicine as well as the Dean of Student Affairs.
Clubs and Organizations Subcommittee
Chair: Student Body VP
Membership in this subcommittee is open only to SGA Delegates. All clubs and student organizations will receive their final approval through the Clubs and Student Organizations Subcommittee. Additionally, the Clubs and Student Organizations Subcommittee will be the final authority in the amount of money that is distributed to all EVMS clubs and student organizations.
Administration Subcommittee
Chair: VP of Administration
Membership in this subcommittee is open to all EVMS Students. The responsibilities of this subcommittee will be to maintain and monitor the Engage website production and content, provide marketing and promotional needs for events and discussions that take place at EVMS through whatever means they deem necessary. The findings are reported to SGA delegates so that the information can be marketed appropriately to all EVMS students.
Finance Subcommittee
Chair: VP of Finances
Membership in this subcommittee is open to all EVMS Students. The responsibilities of this subcommittee will be to evaluate expenses, discuss financial allocations, investments and concerns for the Student Government Association as well as the student body population. Members will work collectively with the Vice President of Finance to maintain and monitor the Student Government Association annual budget through whatever means they deem necessary.
Health Professions Subcommittee
Chair: VP of Health Professions
Membership in this subcommittee is open to all EVMS Students. The responsibilities of this subcommittee will be to advocate, discuss and collaborate on issues pertaining only to the Health Professions Programs. Members of this committee will also work collectively with the Vice President of Health Professions to develop, promote and maintain the Champions Cup events through whatever means they deem necessary.
Community Outreach Subcommittee
Chair: VP of Community Outreach
Membership in this subcommittee is open to all EVMS Students. The responsibilities of this subcommittee will be to identify and collate all of the various community service activities and events that will take place at EVMS through whatever means they deem necessary. The Vice President of Community Outreach will then report their findings to the SGA executives and SGA delegates so that the information can be marketed appropriately to all EVMS students.
Wellness Subcommittee
Chair: VP of Wellness
Membership in this
subcommittee is open to all EVMS Students. The responsibilities of this subcommittee will be to identify and collate all of the various social and wellness events that will take place at EVMS through whatever means they deem necessary. Additionally, the Student Wellness Subcommittee will intimately work with the Student Affairs office to help plan and execute major inter-program EVMS initiatives.
OTHER SUBCOMMITTEES
Many other committees at EVMS desire student input. Student government works to appoint interested students to these committees to serve as representatives of the entire student body. The following students serve on these committees to expand the student voice at EVMS.
Board of Visitors Student Representatives
Academic and Student Affairs Subcommittee of the Board of Visitors requests to have student representation at each one of their quarterly meetings. This important position helps to facilitate communication between student affairs, administrations, SGA and the BOV. This position will be filled by the highest ranking MD and HP students on the SGA Executive Committee.
Building Planning Committee Student Representatives
One MD and one HP student will be selected to serve on the building planning committee which encompasses other EVMS administration, faculty and staff. This position involves assisting the planning committee with ideas, questions and information regarding to the new EVMS building which will be used for MD/HP academics coming in the 2020-2021 academic year.
Academic Capital Projects Advisory Committee
Student representation on the Academic Capital Project Advisory Group: One MD student, who will communicate with MD and HP peers through SGA, to serve. The Group will make recommendations on capital projects (> $10,000) to improve infrastructure aligned with our core missions in the areas of administration, facilities, research, academics, and/or information technology. The Group will meet approximately monthly from October-February.
